The text in the manual suggests the use of initial, medial and final in the
type attribute of the <q> element.  However these are not currently
supported values in the schema.  

I would like to see the value of medial - or some equivalent - be added to
both speech and quote elements along with an attribute which allows me to
tie them back to the original elements.  Possibly mID as an attribute could
be used.
